When i want to create a site using an existing Github provider of the team it gave be an error back in the API. I would recommended using the owner's provider for that team.
Right now i do get an error in the API, not on the page itself as well (network tab selection) "We cannot seem to find the provider "github" linked to your account"
My bad, meant API because of the network tab. The UI is not fine at all. I've DM'd you the screenshot.
